To begin the process, please note that a separate application form is required for each tree or bench request. Even if you are submitting multiple requests, each one needs its own application form. For example, if you are requesting one tree and one bench, you will need to fill out two application forms.

Here’s how to submit your request for a commemorative bench or tree:

  1. Check the availability of your preferred location by reviewing the list of parks that are at capacity and not currently available for commemorative installations.
  2. Download the Commemorative Tree and Bench Program application form.
  3. Submit the completed application via email to commemorativeprogram@toronto.ca.

Application Confirmation

Rest assured that our dedicated staff will provide an initial response within two weeks of receiving your application. This response ensures that your application has been successfully received and placed in the queue for review.

Application Review

Our knowledgeable staff will thoroughly review each application, taking into account site-specific assessments. We strive to respond to applicants within 12 weeks of receiving their applications. In our response, we will:

  • Confirm the installation location or suggest alternative locations, if necessary.
  • Verify the requested wording on the commemorative plaque.

Payment Request and Installation

Once the location for planting or installation is confirmed, applicants will receive a payment request via email. Please note that full payment is required before the planting or installation can proceed.

While we aim to install trees and benches within the calendar year of the request, it’s important to keep in mind that high demand may affect installation timelines. If requests cannot be completed within the calendar year, our team will reach out to you and provide updates.

Due to factors such as weather conditions, tree supply, and park construction, we are unable to specify exact dates for tree planting or bench installation. However, rest assured that donors will receive a written confirmation, complete with a location description and a photo, once the installation is completed.

Certificate and Tax-deductible Receipts

If you indicate your preference on the application form, we will be more than happy to mail you a certificate to recognize your commemorative donation. Additionally, please note that the donation is tax-deductible through the Income Tax Act, Sub-paragraph 110(1) (a)(1v), and tax-deductible receipts are available upon request.

To receive a tax-deductible receipt, please keep in mind the following:

  • Requests for tax-deductible receipts are accepted until December 1 each year.
  • To obtain the Donors’ Declaration form, please email commemorativeprogram@toronto.ca.

Setting Up Payment

To ensure a seamless payment process, donors must have an eFun account set up with the City of Toronto. Here are four ways to get your account set up and ready:

  1. Create an account online. Once you complete the registration, your new Family and Client number will be emailed to you. Remember to keep your Family and Client number in a secure place.
  2. Speak with a customer service representative by calling 416-396-7378, selecting option 1.
  3. Fill out the Family Account Form and email it to pfrcustomerservice@toronto.ca. Please allow five business days for processing.
  4. Fax the completed Family Account Form to 416-392-1551.

To retrieve information on an existing account, follow these steps:

  1. Visit eFun and select “Log in” from the top menu. A pop-up window will open.
  2. On the right-hand side, select “Forgot Login?” and enter the email address associated with your account.
  3. Your Client and Family Number will be emailed to you. If you require assistance, call Client Services at 416-396-7378.

Payment Methods

Once you have an eFun account and have received a payment request, you can provide payment using one of the following methods:

  1. Online through eFun for a convenient and hassle-free experience.
  2. By phone using a credit card. Simply call Client Services at 416-396-7378.
  3. In person by a cheque or money order, payable to “Treasurer, City of Toronto.” Visit one of the following locations:
  • North York Civic Centre, 5100 Yonge St., third floor
  • Etobicoke Civic Centre, 399 The West Mall, second floor north
  • Scarborough Civic Centre, 150 Borough Dr., fifth floor
  • Toronto City Hall, 100 Queen St. W., first floor east
